My husband and I are ready to start our family. How will I know I am pregnant?

There are many signs and symptoms that occur leading a woman to believe she is pregnant. Some are more accurate than others. When we discuss these signs and symptoms they can be classified as presumptive/possible, probable, or positive. It is important to know and understand the differences, so you know if you are truly pregnant.

Presumptive signs are physical changes which occur that could be attributed to other causes.

Probable signs can also be attributed to other causes, but are more likely caused by pregnancy.

Positive signs cannot be attributed to anything other than pregnancy.

Usually the first sign leading a woman to believe she is pregnant is a missed period or menstrual cycle. Pregnancy is the first thing we look for when a woman offers this complaint. However, missed menses may also be caused by illness, hormone imbalance, stress, fatigue, breast feeding, thyroid disease, weight changes, or recently going off the pill.

Nausea and vomiting, that is, morning sickness, is also a presumptive sign, as it can be caused by illness, food poisoning, or stress.

Fatigue is another common first trimester complaint. But fatigue can also be caused by poor sleep, illness, stress, poor nutrition, or overwork.

Frequent urination often occurs with pregnancy, but this can be caused by increased fluid intake, especially diuretic fluids, by illness or diabetes, by a urinary tract infection, or increased stress.

Breast tenderness is common, but could signal the onset of late menses or be from birth control pill. Also, nipples get darker during pregnancy, but this could be a hormonal imbalance or unresolved from a previous pregnancy.

So, you can see that these presumptive signs are not proof positive of pregnancy. Probable signs are similar in that they can also be attributed to other causes, but they are more likely indicative of pregnancy.

A positive blood or urine pregnancy test is considered probable, as the test is not 100 percent positive, it can be false positive. The urine may be very dilute, or the test could be performed incorrectly.

An enlarging abdomen or uterus can occur from bloating, gas, menstruation, or weight gain. These changes can also be caused by fibroids or tumors.

Color changes of the cervix and vagina noted during a pelvic exam (Chadwick’s sign), and softening of the cervix and uterus (Hegar’s sign and Goodell’s Sign) can also be caused by impending menses, which is late but on its way.

A feeling of butterfly fluttering in the lower abdominal region, that is, quickening, could be caused by fetal movement, but also by gas or bowel contractions.

Positive signs are direct consequences of a pregnancy, and cannot be attributed to any other cause. These are your definitive proof of pregnancy.

Visualization of an embryo or gestational sac via ultrasound is indicative of a positive pregnancy. Fetal ultrasound is often performed during the first trimester to confirm the pregnancy.

Fetal heartbeat, either by Doppler at 9 – 12 weeks, or by fetoscope at 16 – 20 weeks, is also positive proof of pregnancy.

Finally, fetal movement (previously described as a probable sign) when felt by your medical provider is further proof of positive pregnancy.

It is best for the health of the mother and the fetus to begin prenatal care early. If you believe you are pregnant, schedule that first visit!

Presumptive signs

These are the symptoms and sensations that, while possibly indicating pregnancy, also could be caused by any number of other conditions. These are also some of the earliest pregnancy symptoms.

  • Absent menstrual periods (amenorrhea)
  • Nausea and/or vomiting (morning sickness)
  • Unexplained fatigue
  • Frequent urination
  • Breast tenderness and changes
  • Excessive salivation (ptyalism)
  • Skin changes
  • A sensation of movement in abdomen (quickening)
  • Probable signs

    Most of the time, these do indicate pregnancy — but, in certain cases, they might be false or caused by another condition.

  • Positive pregnancy test (presence of hCG detected, by blood or urine test)
  • Softening of cervix at six to eight weeks (Goodell’s sign)
  • Bluish coloration of cervix, vagina and vulva at six to eight weeks (Chadwick’s sign)
  • Enlarged abdomen
  • Braxton-Hicks contractions
  • Passive movement of the fetus during an exam (ballottement)
  • Positive signs

    These can’t be mistaken for any other condition. They’re considered absolute evidence that you are, in fact, pregnant. They rely on the senses — sound, sight and touch — as interpreted by your caregiver.

  • Fetal heart sounds heard by Doptone or Doppler
  • Fetus visible on ultrasound
  • Fetal movements felt by caregiver
  • Fetus visible on X-ray, MRI or other diagnostic imaging device (Note: Use of these devices is not recommended during pregnancy)
  • The value of these signs

    Even the presumptive signs are important to know because of the value of prenatal care. As soon as you suspect even that you might be pregnant, start taking extra-good care of yourself: Avoid alcohol and cigarettes, eat well, and start taking prenatal vitamins. Make an appointment with a qualified caregiver to find out what else you need to do to grow a happy and healthy baby.
